Does anyone know how to correct these issues?
Maybe I should just reload the whole OS?
Is there a complete copy of the image that was shipped on the chip that I can reload?

You'll need to flash the device again. There are many versions of the flash laying around, openwrt website is the best place to get one. But it doesn't have luci gui. https://downloads.openwrt.org/barrier_breaker/14.07/ramips/rt305x/openwrt-ramips-rt305x-vocore-squashfs-sysupgrade.bin
